Application

WORKFORCE plus helps young people, ages 14 - 21, overcome barriers which prohibit them from completing their education or getting a job. All of our services are easy to use and available at no cost to you. To apply, download the application (pdf) and return it to your local office.

Eligibility

Bring these documents with you for eligibility purposes:

      v      Birth Certificate

      v      Driver's License - Florida Department of Motor Vehicles

v      Social Security Card (or official verification of social security number) - Social Security Online

v      Authorization to work in the United States (Social Security Card can suffice for this)

v      Proof of Selective Service Registration (if applicable) - Selective Service System

v      Signature of Parent or Legal Guardian on application for youth under age 18

v      Proof of Household income for the past 6 months

ONE of the following possible sources:

o        Pay stubs for last 6 months or

o        UC records (our staff can help pull these) or 

o        Statement employer or employer name/phone number for us to verify your earnings or

o        Public assistance printout or Food Stamp Card with current date or letter from Food Stamp Disbursing or

o        Tax forms (W-2 forms) or

o        Alimony agreement (if within last 6 months) or

o        Personnel records indicating income over last six months
